Javier Sosa, MD

Double Board Certified in Family Medicine and Obesity Medicine, Aesthetic and Wellness Specialist

Javier Sosa, MD, is a double board-certified physician in family medicine and obesity medicine who brings nearly a decade of experience to his practice, Woodlands Primary Healthcare in The Woodlands, Texas. Dr. Sosa provides comprehensive care for patients of all ages, treating common conditions such as respiratory infections, ear infections, and back pain, while also offering preventive examinations, well-child visits, women’s and men’s health services, chronic disease management, weight-loss management, aesthetic medicine, and in-office procedures.

Dr. Sosa completed his family medicine residency at McGovern Medical School at the University of Texas Health Science Center at Houston. In 2024, he earned his Board Certification in Obesity Medicine with Distinction, further expanding his expertise in comprehensive weight management and metabolic health.

Prior to establishing his practice in Texas, Dr. Sosa served as Medical Director for Roosevelt General Clinic in Portales and Emergency Department Medical Director at RGH Hospital in Portales, New Mexico. His exceptional patient care and leadership were recognized when he was voted Doctor of the Year for three consecutive years (2015, 2016, and 2017).

Hernan Martinez, FNP

Board Certified Nurse Practitioner​

Hernan Martinez (pronounced HER n uhn /M or tee neh s) is a medical professional committed to the health of his patients. Originally from Colombia, his medical education and experience is extensive. Hernan earned his medical degree in Colombia at the Universidad Libre Seccional Cali. He completed a fellowship in critical care at the Hospital Universitario Del Valle Evaristo Garcia in Colombia. He is a Foreign Medical Graduate (FMG) and a member of the American Association of Clinical Endocrinology (AACE).
Hernan moved to the U.S. in 1998. Hernan obtained his master degree as a Family Nurse Practitioner at the Florida International University. Hernan also has experience in the treatment of diabetes mellitus and thyroid disease while undergoing an endocrinology program (1 year) at the time he was working as a family nurse practitioner at the Cleveland Clinic in Florida. Hernan treats children, adolescents and adults. Hernan speaks Spanish fluently as well.
On a personal note, Hernan enjoys spending time with his spouse and two children. He also participates in lronman triathlons and enjoys working out and living a healthy lifestyle.

Jaime Ochoa, APRN, FNP, ENPC

Board Certified Nurse Practitioner

Board Certified by the American Academy of Nurse Practitioners (MNP), Jaime Ochoa, FNP-C, is a bilingual Family Nurse Practitioner born and raised in Houston, TX. He obtained his Bachelor of Science in Nursing (BSN) from The University of Texas Medical Branch in Galveston and then attended Cizik School of Nursing at The University of Texas Health Science Center at Houston to obtain his Master of Science in Nursing (MSN) with a focus in Family Medicine and a Post-Master’s certification in emergency medicine and trauma.
Jaime Ochoa, FNP-C is currently a member of the American Academy of Emergency Nurse Practitioners (MENP) with Advance Cardiovascular Life Support (ACLS) and Pediatric Advance Life Support (PALS) certifications.
In addition to his strong emphasis on health promotion, risk reduction, and disease prevention, Jaime Ochoa, FNP­C can treat every day illnesses including, ear and throat infections, respiratory infections, urinary tract infections, and acute musculoskeletal injuries, while also offering well-child visits, women’s and men’s health care, and management of chronic diseases.

Danaisy Reyes Morales, FNP

Board Certified Nurse Practitioner​

originally from Matanzas, Cuba, is a doctor who graduated in 2010 and obtained her specialization in family medicine and Emergency Care in 2013. During those years, she worked in the care of children, adults, pregnant women, and elderly patients. In 2013, she participated in a medical mission in Venezuela and later in Colombia, where she provided her services as a Family Medicine doctor.
She also earned her Bachelor of Science in Nursing (BSN) from The University in Puerto Rico and worked in the field of home health for over 3 years. Subsequently, she pursued her degree as a family nurse practitioner FNP-C at the University of Georgia South University.Since obtaining her license, Danaisy has focused on providing primary care services, offering treatment to children, adults, and elderly patients, managing chronic pathologies, acute conditions, and emphasizing disease prevention.
In her free time, Danaisy enjoys spending quality moments with her husband and two daughters, and she values her close-knit family.
